Oklahoma sign errors

Below is a table which I am compiling of road sign errors in Oklahoma. Each error is numbered sequentially and information on its location and the description of the error is included. I am not affiliated with ODOT, and this project is more for kicks than anything. However, if ODOT stumbles across this and corrects all the errors, well, thanks, and good job, guys. ;)

To be listed here, the error must be more than stylistic. This means that aesthetic errors (such as off-center placement of shields, font distortion) or temporary maintenance issues (damaged, bolt came loose and sign is upside down now) will not be shown here. Instead, the error must be something more along the lines of the following:

Here is a breakdown of what the columns mean.

No.
Sequential number for reference. Links in this column will lead to a photo or a page with a photo and more information about the error.
Submitter, date
Who discovered the error and/or submitted it to me, and the date they observed the error.
County, Location, Hwy.
Location of the error. The number after the county name is the ODOT sequential county number (Adair is 1, Woodward is 77).
Div.
ODOT division responsible for the area where the error is. If a "T" appears in this column, it is on a toll road (and thus the responsibility of the Oklahoma Turnpike Authority, not ODOT). If a dash appears here, it is the responsibility of a local government.
Remarks
Brief description of the error.
Status, last update by, date
Last observed status, who did the observing, and the date. Active means the error is not known to be fixed yet; Confirmed means the sign has been observed again and it is still unfixed. Replaced, Patched, Installed, and Fixed mean the error has been corrected (and the details about how that was done). Note that the date listed here doesn't necessarily correlate with the time that ODOT took to fix the error; they could have fixed it the next day after it appeared here and no roadgeek happened to observe the sign until the date shown.

If you know of an error in Oklahoma which isn't listed here, contact me. New error reports should have all the information listed below. If you took a picture of the error, feel free to attach it, or send me a link to where you uploaded the file (like your personal site, or Flickr). If you have an update to a previous error report (confirming it still exists, or reporting it fixed), again, please contact me (this is probably actually more important than reporting new goofs because we're giving ODOT credit for fixing their mistakes). In all reports, please include the date you observed the sign if you can. Hint: if you took a picture of the error, most digital cameras will record the date the photograph was taken in the EXIF metadata embedded in the JPEG.
